Adding an image?

Ok, sorry, I must be an idiot :(

Let's say I am starting from scratch, how do I ADD an image? I see how to replace, but what about adding more images? Am I missing the obvious or does this not exist?

With the image gallery displayed, right click and select "Add an image"
